Lifeline Reactivation Form

As you may be aware, the Federal Lifeline Program is an FCC benefit program that provides a discount on phone service for qualifying low-income subscribers. The benefit is limited to one monthly service discount and one connected device discount per household. Households can only receive the Lifeline benefit from one service provider at a time. You may transfer your Lifeline benefit to another provider at any time, but you cannot transfer your benefit to another person.

Excess Telecom has contracted with IM Telecom to provide Lifeline service and benefits and enables you to apply those benefits to Excess Telecom branded plans. By executing this form, you agree to transfer your Lifeline benefit to IM Telecom and to apply your Lifeline discount benefits to Excess Telecom branded services. Your Lifeline discount benefits will no longer be applied to service retained from the transfer-out provider. You may be subject to the transfer-out provider's undiscounted rates as a result of the transfer if you elect to maintain service with that provider.

You authorize Excess Telecom and its contracted partners, for the purpose of applying for, determining eligibility, enrolling in, and seeking reimbursement under the Federal Lifeline service benefit to collect, use, share, and retain your personal information, and to provide it to USAC to ensure proper administration of the program services and/or connected device. Failure to provide this consent will prevent you from being able to transfer your Lifeline benefits back to Excess Telecom at this time.
